Pathways HealthConnect Program
The Pathways HealthConnect program connects individuals (Age 18+) to the services and support needed to live a healthier lifestyle. Community health workers can assist participants in addressing the following needs:
- Medical, dental, vision, and hearing
- Behavioral health
- Housing, Food, and Clothing
- Education
- Employment
- Financial
- Transportation
- and many more.
