Pathways HealthConnect Program 

The Pathways HealthConnect program connects individuals (Age 18+)  to the services and support needed to live a healthier lifestyle. Community health workers can assist participants in addressing the following needs:

• Medical, dental, vision, and hearing
• Behavioral health
• Housing, Food, and Clothing
• Education
• Employment
• Financial
• Transportation
• and many more.
